    The Potential Benefits of Introducing Informal Recyclers and Organic Waste Recovery to a Current Waste Management System: The Case Study of Santiago de Chile

    Chile has experienced rapid economic growth in recent decades. However, this economic growth has been accompanied by a huge increase in waste generation. Although the country has historically put emphasis on appropriate final disposal of waste through landfill, the environmental problems generated by this activity have shown that it is necessary to improve the national solid waste management approach. Santiago, the capital of Chile, generates about 43% of the national solid waste. A study conducted by the Ministry of the Environment in 2011 found that 14% of the waste generated in the capital is recycled, mostly thanks to local campaigns and collection by the informal sector (scavengers). While in 2009 the government set a target to recycle 25% of municipal inorganic waste by 2020, there is no information on the implementation process to reach this target. Most importantly, the law has not established specific recycling rates for target materials, and, has not taken into consideration organic waste recovery, which accounts for 48% of the total waste stream. In order to meet the government target and at the same time promote organic waste recovery, this study proposes and evaluates the environmental impacts of different viable alternatives for municipal solid waste collection, treatment and recovery by using Life Cycle Assessment (LCA). The proposed scenarios range from the current situation to the introduction of organic waste recovery and the inclusion of informal recyclers in the collection process. By considering the investment and treatment costs of each alternative, the study identifies the most effective scenario in terms of avoided pollutants per cost ratio. Finally, the study analyzes the feasibility of the implementation of the selected scenario by indicating benefits and challenges. The results of the scenario evaluation suggest that the scenario with the highest recovery of inorganic and organic materials, coupled with the participation of the informal collectors, will have a positive impact not only in terms of meeting and surpassing the government goal, but also in the reduction of CO2eq emissions. This scenario can reduce by approximately 3.5% the national CO2eq generated, with a cost of $14.1/ton. Moreover, the potential reductions of CH4 account for 8.5% of the national CH4 emissions and 24.5% of the national waste sector CH4 emissions

    Prótesis dentales y patologías en adultos mayores de un centro de salud, Trujillo – 2021

    The present investigation held as a general objective to correlate the adaptation of dental prostheses and the pathologies of the oral mucosa in elderly patients treated in a health center. The type of research was basic, relational level, also has a non-experimental design with a cross-section. Through probability sampling, the sample consisted of 60 older adults with dental prostheses treated in the dentistry office of a health center in the city of Trujillo. The technique used was the survey technique, its instrument being the questionnaire, which was previously validated by experts in the field. Concluding that: there is a correlation between the adaptation of the prosthesis and the pathologies since it resulted in 0,000 in its chi square, an amount less than the 0,05 level of significance. Likewise, it is realized that the level of correlation is high, since the result of the contingency coefficient was 0,666, suggesting that, if there is a poor adaptation of dental prostheses, greater pathologies will result in the oral mucosa of older adults.La presente investigación sostuvo como objetivo general correlacionar la adaptación de prótesis dentales y las patologías de la mucosa oral en pacientes adultos mayores atendidos en un centro de salud. El tipo de investigación fue aplicada, de nivel relacional, asimismo tiene un diseño no experimental con corte transversal. Mediante el muestreo probabilístico, la muestra estuvo conformada por 60 adultos mayores portadores de prótesis dental atendidos en el consultorio de odontología de un centro de salud en la ciudad de Trujillo. La técnica que se utilizó fue la técnica de la encuesta, siendo su instrumento el cuestionario, que previamente fueron validados por expertos de la materia. Concluyendo que: se aprecia una correlación entre la adaptación de prótesis y las patologías ya que tuvo como resultado en su chi cuadrada un 0,000, un monto menor al nivel de significancia del 0,05. Asimismo, se observó que el nivel de correlación es alto, dado que el resultado del coeficiente de contingencia fue de 0,666, dando a entender que, si existe una mala adaptación de prótesis dentales, resultarán mayores patologías en la mucosa oral de los adultos mayores

    High prevalence of chitotriosidase deficiency in Peruvian Amerindians exposed to chitin-bearing food and enteroparasites

    The human genome encodes a gene for an enzymatically active chitinase (CHIT1) located in a single copy on Chromosome 1, which is highly expressed by activated macrophages and in other cells of the innate immune response. Several dysfunctional mutations are known in CHIT1, including a 24-bp duplication in Exon 10 causing catalytic deficiency. This duplication is a common variant conserved in many human populations, except in West and South Africans. Thus it has been proposed that human migration out of Africa and the consequent reduction of exposure to chitin from environmental factors may have enabled the conservation of dysfunctional mutations in human chitinases. Our data obtained from 85 indigenous Amerindians from Peru, representative of populations characterized by high prevalence of chitin-bearing enteroparasites and intense entomophagy, reveal a very high frequency of the 24-bp duplication (47.06%), and of other single nucleotide polymorphisms which are known to partially affect enzymatic activity (G102S: 42.7% and A442G/V: 25.5%). Our finding is in line with a founder effect, but appears to confute our previous hypothesis of a protective role against parasite infection and sustains the discussion on the redundancy of chitinolytic function

    Programa de Atención al Ictus en Aragón (PAIA). Estrategia del cambio y resultados en el periodo 2009-2014

    Introducción En 2008, Aragón tenía tasas de morbimortalidad y discapacidad por ictus superiores a las del conjunto de España. Se estableció la necesidad de desarrollar un Programa de Atención al Ictus (PAIA). Material y métodos Damos a conocer la dinámica de planificación, implantación, evaluación y mejora que se ha desarrollado entre los años 2009-2014 y sus resultados a 5 años. Resultados Se ha mejorado en la estructura, en los procesos y en los resultados, con mejoría en los indicadores clave de la asistencia (audit 2008-2010-2012) y otros: tasa ictus 2013: 2, 07 (2008: 2, 36); 78% ictus atendidos en áreas/unidades en 2014 (30%, 2008); tasa fibrinólisis 8, 3% en 2014 (4, 4%, 2010); fibrinólisis hospitales secundarios (30% total); fibrinólisis con teleictus 9%; descenso de la mortalidad por ictus, 38%; años de vida prematura perdidos 67, 7 (2013)/144 (2008); capacitación de enfermería, desarrollo de la neurosonología, trabajo en red, con protocolos y buenas prácticas compartidos entre sectores sanitarios, etc. Conclusiones La gestión por procesos y equipos multidisciplinares desplegados en una distribución territorial integral, con protocolos y referencias establecidas y una dinámica de evaluación y mejora continua, ha demostrado ser una herramienta potente para garantizar la calidad y la equidad. El PAIA, por su dinámica de mejora sostenida y la implicación de los clínicos, es un buen ejemplo de gestión clínica y trabajo en red. Introduction: In 2008, stroke mortality, morbidity, and disability rates in Aragon were higher than the average in Spain. These data underscored the need to develop a stroke care programme (PAIA). Material and methods: We present the dynamics of planning, implementation, evaluation, and improvement developed between 2009 and 2014 as well as the results of the PAIA after that 5-year period. Results: Structure, processes, and outcomes have improved with reference to the key indicators of healthcare (audit: 2008, 2010, 2012) among others: stroke rate in 2013 was 2.07 (2.36 in 2008); 78% of strokes were managed in stroke units in 2014 (30% in 2008); rate of fibrinolysis was 8.3% in 2014 (4.4% in 2010); fibrinolysis was administered in secondary hospitals (30% of the total); fibrinolysis was administered by Telestroke in 9%; stroke mortality decreased (38%); 67.7 years of potential life lost (YPLL) in 2013 (144 in 2008); nurse training; development of neurosonology; networking; sharing protocols and best practices between health sectors, etc. Conclusions: Integrated process management and multidisciplinary teams distributed and deployed over an entire territory with established protocols, references, evaluations, and continuous development, have been proven powerful tools to ensure both quality and equality. The PAIA is a good example of clinical governance and networking due to its dynamic and sustained improvement and cooperation between clinicians
